1. Description: Fareston is indicated for treatment of metastatic breast cancer in postmenopausal women with estrogen-receptor–positive or unknown tumors. Fareston was approved by the U.S. Food and Drug Administration for use in October 1995.

3. Drug Mechanism: Fareston is a medicine that blocks the receptors for the female hormone oestrogen in the body. This prevents oestrogen-induced stimulation of DNA production and hence cell growth in breast cancer.
Cancers form when some cells within the body multiply uncontrollably and abnormally. There are two types of cancer. Solid cancers where a lump forms e.g. the bone, breast, muscle, brain cells etc. divide and multiply abnormally. The second type are leukaemias and lymphomas where the blood cells divide and multiply abnormally.
Other characteristics of cancer besides uncontrolled growth include the ability of these abnormal cells to invade other tissues next to them or to break away from the original site, travel through the blood or lymph, and establish a new cancer at a different site of the body. These are called metastases.
Breast cancer can be classifed into oestrogen sensitive and oestrogen insenstitive cancers. Most breast cancer is sensitive to oestrogen. Sensitive cancer cells need oestrogen to stay alive and removal of oestrogen from the body or stopping any circulating oestrogen getting to the cancer cells can be effective at controlling or killing hormone sensitive breast cancers.
Therefore, Fareston, by blocking the receptors for oestrogen will cause the cancer to shrink in size or limit the spread of the cancer to other parts of the body.
4. Dosage Form: Fareston is usually taken once a day (60 mg). Follow your doctor's instructions. Take each dose with a full glass of water. Toremifene can be taken with or without food. Toremifene should be taken at the same time each day whenever possible.
5. Drug Interactions: The effect of blood thinning medications such as nicoumalone and warfarin may be enhanced and so there is an increased risk of bleeding.
The antiepileptics carbamazepine, phenobarbital and phenytoin may reduce the levels of toremifene in the blood.
There is an increased risk of hypercalcaemia if thiazide diuretics are used with toremifene.
6. Pregnancy: This medicine should not be used in pregnancy. Although Fareston is not generally prescribed for premenopausal women, it is important that patients be sure they are not pregnant before starting treatment with this drug. Seek medical advice from your doctor.
7. Nursing Mothers: There is no information available about the safety of this medicine during breastfeeding. For this reason, the manufacturer states that it should not be used during breastfeeding. Seek medical advice from your doctor.
8. Dietary and Alcohol Considerations: Avoidance or limiting the use of caffeine, nicotine, and alcohol may help decrease hot flashes and sweating associated with the use of Fareston.

Headache
Changes in skin pigmentation
Fatigue
Sweating
Weight gain
High blood calcium level (hypercalcaemia)
Dizziness
Nausea and vomiting
Abnormal vaginal bleeding
Hot flushes
Excessive fluid retention in the body tissues, resulting in swelling (oedema)
Discharge of white fluid from the vagina (leukorrhoea)
Pain

10. Storage: Keep this medication in the container it came in, tightly closed, and out of reach of children. Capsules should be stored at room temperature (77°F (25°C)) and away from excess heat and moisture (not in the bathroom). It is okay for brief periods in temperatures between 59-86°F (15-30°C). Throw away any medication that is outdated or no longer needed.
